To facilitate this, we have a history form, which we request the patients to fill before the detailed consultation. We intend to select the best possible medicine for you. This includes your reactions to various factors, your past and family history and your mental makeup. This information enables us to select the remedy that removes your sickness. The medicine also makes you well as a whole person. Thus, the History form written by you becomes one the greatest assets in treating you.
